- AGREED PAYMENT TERMS:
- ${{ payrate }} per hour pay to {{ name }}.
- $25 placement fee pay to Happy Dental Jobs at the end of the shift.
- TERM OF AGREEMENT: This Agreement shall become effective immediately upon Client’s execution of the Agreement where indicated below. It shall remain in effect until it is terminated by either of the Parties.
- HIRING, COMPENSATION & PAYMENT: Workers referred by Happy Dental Jobs, once hired, are the Client's employees or independent contractors. The Client is solely responsible for all wages, taxes, benefits, insurance, and other compensation. Salaries or wages must align with the agreed pay of $9 per hour. Clients must pay Happy Dental Jobs based on the agreed rate. The doctor is responsible for all payroll taxes, employment expenses, and mandatory insurance. The Client manages all tax responsibilities and forms for each worker. Workers should be paid at the end of their shift. Workers are not employees or contractors of Happy Dental Jobs, and no employer-employee relationship exists between Happy Dental Jobs and any Worker.
- REFERRAL FEES:
The Client agrees to pay Happy Dental Jobs upon invoice receipt for referred Worker(s) as per the assignment terms.
- Temporary placements cost $25.00 per shift, payable upon shift completion.
- Cancellations after 7:00 PM the day before placement or on the same day will incur a $25.00 fee.
- Assignments have a 4-hour minimum. If cancelled while the candidate is en route, a 2-hour pay for half-day or 4-hour pay for full-day assignments is due to the candidate.
- HOLD HARMLESS: Workers hired through Happy Dental Jobs solely bear responsibility for job performance and contract completion. They are not Happy Dental Jobs' agents or employees. Happy Dental Jobs shall not be liable for any loss, injury, or damage caused by any Worker.
- PERSONNEL QUALIFICATIONS: The Client holds ultimate responsibility for determining the qualifications of Workers referred by Happy Dental Jobs.
- WORKING ENVIRONMENT: The Client commits to a safe, harassment-free environment and will adhere to all relevant employment laws. The Client confirms having worker’s compensation insurance for their practice.
